How I may use and disclose information
I may use or share health information for treatment, payment, and health care operations. This can include coordinating care with other providers, billing you or your health plan, managing the practice, improving quality, and reviewing professional performance.
I may also use or disclose information when permitted or required by law, including for public-health activities, health oversight, judicial or administrative proceedings, certain law-enforcement purposes, workers’ compensation, preventing or reducing a serious threat to health or safety, and reporting suspected abuse, neglect, or domestic violence as required.
Other uses and disclosures, including most uses of psychotherapy notes, marketing, and sale of health information, generally require your written authorization. You may revoke an authorization in writing, except to the extent I have already relied on it.
Your rights
- Request access to or a copy of your health record, with limited legal exceptions.
- Ask me to correct information you believe is incorrect or incomplete.
- Request confidential communications or reasonable alternative ways to contact you.
- Ask me to limit certain uses or disclosures; I am not always required to agree.
- Request an accounting of certain disclosures.
- Receive a paper copy of this notice at any time.
- Choose someone with legal authority to act for you.
If you pay in full out of pocket for a service, you may ask me not to disclose information about that service to your health plan for payment or health care operations. I will honor that request when legally required.
My communications
I may contact you about appointments, treatment alternatives, or health- related services that may interest you. Tell me if you prefer a particular phone number, address, or communication method.

I may change this notice and make the revised notice effective for all health information I maintain. The current notice will be available on this website and upon request.
